This template is a useful way to record information following a RTW interview with an employee who is returning to work after a mental health issue. You can use the template to record:
the employee’s personal details
policies relating to sick leave and time off
the impact of symptoms or medication on work
reasonable adjustments
effects of reasonable adjustments on pay and entitlements
The plan also covers who (if anyone) to tell and what to tell them, what to do if the employee becomes unwell, and the agreed date of the next meeting.
