This template is a useful way to prepare and document a RTW plan for an employee returning to work after a mental health issue. The RTW coordinator and the employee can work together to decide:
the employee’s RTW goals
starting number of work days and hours
target number of work days and hours
breaks and other adjustments
duties.
There is also space to record the employee’s personal details, and comments and actions during the plan period. The template covers a 4-week period, but this can be shortened or extended as appropriate.
