Tool 6: Supplier’s Report – Template

You can use this template to evaluate the performance of the suppliers delivering services to your organisation. It has 2 sections:

• The supplier completes section 1 – This is data about use of the service (e.g. number of users, feedback from users).
• Your organisation completes section 2 – This section evaluates the information provided by the supplier (e.g. proportion of workforce using the service tracked over time, review of feedback from users).
